White Oak Care provides a range of supported living services across the UK, each shaped around the individual we support. Whether the need is help with daily living, recovery from a mental health episode, or specialist support for complex co-occurring conditions, our approach stays the same: listen first, plan together, deliver consistently.
We work closely with families, social workers, NHS teams, and commissioners to make sure every placement is the right one. If we're not the right fit, we'll tell you honestly and help you find who is.

Who we support
We support adults aged 18 and over across the UK whose needs include, but are not limited to:
- Learning disabilities (mild, moderate, and severe)
- Autism spectrum condition, including non-verbal communication needs
- Mental health diagnoses, including step-down from inpatient settings
- Dual diagnoses and behaviours that challenge
- Physical and sensory disabilities alongside other support needs
- Transitions from children's services, family home, or residential care
- Short-term and respite placements

Free assessment
We meet the person, the family, and any professionals involved. We listen before we plan.
Personalised support plan
Built around goals, routines, communication, and what a good day looks like. The person leads it.
Matched team
Support workers are matched to the person's needs, preferences, and personality not just the rota.
Move-in or service starts
Whether at home or in one of our properties, support begins with the right people, trained for the right needs.
Ongoing review
Regular reviews with the person, family, and professionals. The plan moves as the person moves.

We make referrals straightforward. Send us the assessment, the support plan, and any risk information you have, and we'll respond within one working day. Where placements are urgent, we'll tell you honestly whether we can meet the need and how quickly.
